Use TDSSKiller to scan for the TDSS file system:
Click Change Parameters. Select TDLFS File System. Click OK. Click Start Scan.

Another way is to use WinPE to wipe the forth partition, FixMBR to wipe the MBR, and TestDisk to restore the valid partitions. DSRFix can then be used to restore the Dell Recovery Partitions.
Loading PBR for Descriptor 2 Error – BIOS Settings
The error was only half of the original issue. I probably forgot to mention the Dell alarm beeps. A series of fast beeps, like an alarm or some kind of error when it started up. Dell diagnostics all came back clean including the full HDD scan.
The solution: BIOS setting change from FAST BOOT to THOROUGH.
